This personalized service includes family collaboration to provide an experience unique to the resident. By identifying needs and characteristics of the resident’s personality, activities, meals, and services are offered that the resident will find most meaningful, personal, and engaging.

The Tapestry House encompasses four signature programs that aim to embrace the individual preferences of each resident. First, the Connectivity activities program ensures varied and engaging days for each resident. Group and individual activities are planned by experienced recreational therapists. The second program is Dining with Purpose. This concept focuses on making mealtime the best experience it can be. Thirdly, the Safety First Fall Management program maintains the quality of life for residents by minimizing fall risks. It implements a fall prevention plan unique to every resident.

Finally, the Seasons Memory Care program is unique to Tapestry House. This program requires all staff to go through specialized training in providing memory care. This resident-centered program follows memory care researcher and educator Teepa Snow’s Positive Approach to CareÒ. This prospectus relates the stages of memory loss patients to six colorful gemstones in the GEMSÒ Brain Change Model. This well-developed program is itself a jewel and utilizes in-depth scientific concepts to navigate caregivers and patients through the difficult journey of memory care. At the beginning of each day, the underlying goal is to have a joyful day, every day, for both patients and caregivers.

Additionally, Tapestry House partners with a local physician’s group to provide residents with optional on-site medical service for their convenience. This group visits the community on a regular basis to ensure residents receive the medical care they need. Physical and occupational therapies are also available on site.
